New Client Intake Form
Please keep in mind that I am in mom, wife and glam-ma and so I do have a life outside of the salon. With that, I do not accept walk-ins and if (by chance) I don’t have an appointment at a certain time, I may close the salon. That is why it is important that you not only have an
appointment, but that you’re on time.
The salon entrance is just inside the gate on the west side of the property. There are stairs, so unfortunately the salon is not wheelchair accessible.
You are welcome to park in the driveway.

Cancellation/ No-Show Policy:
As a stylist I only make money if there’s a client in my chair. So if someone cancels without enough time to fill the spot, or if someone no-shows I don’t make any money. This is not only unfair to me, but also unfair to other clients who would’ve liked an earlier appointment. Also, cancelations and no-shows effect inflation (because prices then have to be raised to account for such situations) and we all like to see prices staying down as much as possible.
So with that said it is necessary to have a Cancellation/ No-Show policy in order to protect everyone. Here’s my policy at Beauty Empowered:
• All cancelations and/or reschedules must be done 48 hours prior to the appointment or it counts as No-Show.
• If you’re 15 minutes late, you will be counted as a No-Show and will need to reschedule.
• All No-Shows will result in the following:
1. First time will receive a $50 No-Show fee charged to the card in file.
2. Second time will result in a fee equal to 50% of the scheduled service or $50 (whichever is greater).
3. Third time will result in a fee equal to 75% of the scheduled service or $50 (whichever is greater).
4. Fourth time will result in a fee equal to 90% of the scheduled service or $50 (whichever is greater).
Additionally, after the fourth No-Show you will no longer be allowed to schedule appointments at Beauty Empowered.

First Appointment & New Client Policies:
As a new client a $25 deposit is required to secure your first appointment. This deposit will go toward the balance of your service the day of your appointment. The deposit is only able to be paid via credit card. However, the day of your appointment you’re welcome to pay for services using cash, credit/debit card or Venmo.
Release from liability:
As a licensed Hair Designer I will always do my absolute best. However, I am human, mistakes happen and there are also unforeseen surprises (if you will). Sometimes, client’s hair chemistry makeup reacts differently then expected, professional chemicals react to client’s at-home regimens, and/or clients are sadly not always 100% truthful about their hair’s history. Again, I will always do my best and if a mistake is made and the fault is mine, I will do what I can to
rectify the situation. In situations where your hair might not turn out exactly as you’d hoped, you release me from any claims of liability.
